Order Form

Okay, so you want a header- now what? Well, the answer is to fill out the order form below! After you have dont that, I will work on your header and hopefully have it done within one week of the date the order was submitted! Once I complete your header, I will email you to tell you it is finished, but my email will not allow me to send attachments, so you will have to go to the Pick-up page to get your header and start using it!!!

Here’s the order form:

1. What color do you want the background to be?

2. What color do you want the text to be?

3. What text do you want on the header?

4. What pictures do you want on the header?

5. What site will this be for?

6. How big do the dimensions need to be? (Ex. 795 x 150 pixels)

7. What is your email address?

8. Do you have any suggestions for a layout? (Ex. Text crooked on side, picture on far left side tilted to right side)

So, do you see what I mean by the answers for the order form? I will put the actual header described above on the Pick-up page as soon as I can figure out how to put a picture on with WordPress.
